The most common reason for underwhelming results from jowls treatment is a mismatch between the treatment used and the underlying cause of the jowling.
A sagging jawline is not merely “loose skin.” Depending on your age, genetics, and structural makeup, your jowls may be caused by shrinking bones, weakened deep facial anchors, sliding fat pockets, or thinning skin. Achieving a sharp, natural-looking lower face requires diagnosing which layer of your face is contributing to the jowling and selecting a treatment calibrated specifically to that depth.
Table of Contents
ToggleUnderstanding which layer is responsible for your jowling is the first step toward effective treatment. The table below outlines the root causes of jowls by different layers of your skin and why it is particularly significant for Asian patients in Singapore.
| Root Cause | What Happens | Why It Happens | Why It Matters More For Asians (Local Factors) |
|---|---|---|---|
| Loss of Bone Structure | As we age, our jaw and chin loses volume and rotate backward. | Continuous bone loss along the lower jaw reduces the foundational scaffold supporting overlying soft tissue1. | Asian jawlines naturally feature shorter mandibles or shorter chin compared to Caucasian’s; even minimal bone loss quickly triggers visible jowling. |
| Retaining Ligament & SMAS (Superficial Musculoaponeurotic Layer) Laxity | Facial retaining ligaments (structural anchors) and the SMAS sheet (supportive fascial layer) slacken and lose tension. | Age-related weakening and stretching of these tissues cause midface tissues to slide downward under gravity2,5. | Asian facial structures often rely heavily on strong ligament suspension to hold up the deep tissue; anchor weakening leads to pronounced lower-face sagging. |
| Subcutaneous Fat Pad Migration | Deep and superficial facial fat compartments shift out of position as support weakens over time. | Downward displacement of the fat pad causes localised bulge accumulation along the jawline border3. | Prominent malar (cheekbone) fat pads in Asian faces add significantly heavier soft-tissue volume to the lower jawline when midface fat descends. |
| Collagen Depletion | Skin loses collagen, leaving the lower cheeks thin, crepey. | Intrinsic ageing combined with extrinsic damage degrades the skin’s elastic fibre network over time. | Singapore’s intense, year-round tropical UV exposure significantly accelerates photo-ageing and collagen loss. |
The appearance of a jowl can give some clues about what is contributing to it, although most people have a combination of factors.
| What You Notice | What It May Indicate |
|---|---|
| Loose or crepey skin around the jaw | Skin laxity |
| Lower cheek looks heavier or has shifted down | Weak ligament with fat pad descent |
| Distinct fullness beside the chin | Fat pad descent/receded chin |
| Flatter cheeks + heavier-looking lower face | Volume loss and fat pad descent |
| Several of these features | Multifactorial jowling |
These are only clues, not a diagnosis. A proper assessment should look at the skin, fat distribution, tissue position, chin projection and overall facial proportions before deciding which treatment is appropriate.
At Astria, treatment for jowls is approached not with generic, ‘one-sized fits all’ approaches, but through a rigorous, cause-targeted diagnostics.
Rather than treating symptoms in isolation, every patient undergoes a detailed facial assessment to isolate the precise combination of bone resorption, SMAS laxity, fat migration, or dermal thinning contributing to lower-face sagging.
Because lower-face ageing is rarely confined to a single layer, Astria formulates custom treatment plans. High-intensity focused ultrasound (Ultraformer MPT) contracts deep tissues, RF microneedling (Morpheus8) remodels stubborn fat pads, and structural biostimulators or dermal fillers restore lost support.
Protocols are specifically calibrated for Asian facial frameworks – focusing on restoring chin projection and contouring without adding unwanted bulk to the face.
Treatments prioritize patient comfort through extended topical numbing protocols and focus on long-term neocollagenesis for natural, progressive rejuvenation.
Because jowling can have several underlying causes, treatment should be matched to what is contributing most to the loss of jawline definition.
| Targeted Concern | Primary Treatment | Root Cause Targeted | How It Works |
|---|---|---|---|
| Rebuilding Lost Bone Support | Fillers and/or Biostimulators | Bone resorption (receding chin and blunted jawline angle) | Injected directly onto the jawbone surface to restore lost chin projection and jawline structure, stretching sagging lower-face tissue taut across the newly projected bony scaffold. |
| Tightening Sagging Ligaments & SMAS Fascia | Ultraformer MPT | SMAS and retaining ligament laxity | Delivers targeted micro-focused ultrasound into the deep SMAS layer (the same layer targeted in surgical facelifts) to tighten sagging fascial tissue without damaging the skin surface. |
| Shrinking Shifted Fat Pads | Morpheus8 and/or Ultraformer MPT | Subcutaneous fat pad migration and localised lower-face bulges | Morpheus8 delivers radiofrequency heat through microneedles directly into excess unwanted fat compartments while contracting surrounding fibrous tissue walls by adjusting the depth required. Ultraformer MPT’s targeted fat reduction setting can bulk heat specific areas to melt away excess fat tissue in the jowls. |
| Restoring Thin, Sagging Skin Matrix | Volnewmer and/or Collagen-Stimulating Injectables (PDLLA or ECM Skin Booster) | Dermal collagen depletion and skin surface crepiness | Volnewmer delivers volumetric thermal energy into the skin to contract existing collagen fibres and trigger new elastin production, while Collagen-Stimulating Injectables stimulate natural collagen production to increase elasticity of your skin. |
An Asian lady in her 40s came with lower-cheek heaviness and persistence of her jowling approximately six months after undergoing a thread lift overseas. On reassessment, the more prominent feature was lower-cheek fullness, rather than skin laxity alone. In this situation, simply adding another lifting treatment would not have addressed the underlying volume contributing to the heaviness of the lower face. Instead, the treatment strategy was changed to first reduce the excess lower-cheek volume using Ultraformer MPT’s fat reduction setting followed by tightening the SMAS layer. After subsequent sessions, she noted a reduction in lower-cheek heaviness with improved definition along the jawline.
The important lesson is not that one treatment is superior to another, it is that the same-looking jowl can have very different underlying causes. The goal is to identify the dominant cause first then choose the treatment that addresses it.
“A jowl is not a diagnosis. It is a visible sign of several possible changes occurring within the lower face.”
This is why two patients with apparently similar jowls may require completely different treatment strategies.
Conclusion
Achieving a defined, natural jawline is not about choosing the newest device, but about accurately diagnosing which anatomical layer needs support. Selecting a treatment matched to your specific anatomical profile is the key to lasting, predictable results.
If you are experiencing lower face sagging or are unsatisfied with previous treatments, schedule an in-person consultation with Astria Medical Aesthetics to map your facial architecture and design a personalised, multi-layer treatment protocol.
The timing and appearance of jowling are influenced by several factors, including facial structure, genetics, skin quality, fat distribution and age-related changes in supporting tissues. Two people of the same age can therefore develop very different lower-face changes.
There is no universally better treatment. HIFU (High-Intensity Focused Ultrasound) may be considered when deeper tissue laxity is an important contributor, while RF (Radiofrequency) can be useful for improving skin and soft-tissue firmness. The choice depends more on what is causing your jowl than on which technology is considered newer or stronger.
Jowling does not necessarily mean you have gained facial fat. Age-related changes in fat distribution, tissue position, collagen and underlying facial support can make the lower face appear heavier even when your overall weight has remained stable. This is why simply trying to lose weight or reduce facial fat may not address the actual cause.
Most of our patients report minimal discomfort levels due to medical-grade numbing cream being used.
It depends. Adding volume directly into an already-full jowl may increase heaviness. In contrast, carefully placed filler in selected structural areas, such as the chin or prejowl region, may improve the transition and overall facial proportions in appropriate patients. The objective is not simply to “fill the jowl”.
